RE: Settlement Payments in the Galilee Class Action Lawsuit
Dear Class Member:
The claims administration process has finally been completed and the Court has authorized CMM to start dispersing settlement checks to all Class Members that had their claim approved in this case. This notice is intended to update each of you regarding the timing, method, and requirements for picking up your settlement check. If you are receiving this Notice, the Court approved your claim.

The settlement checks will be available to be physically picked up starting on January 9th, 2023, at 10:00 a.m. The settlement checks will be available for pickup at the Shelby County Courthouse located at 140 Adams Avenue, Memphis, TN 38103, in Room 330. Room 330 is marked on the door as the Tennessee Supreme Court room. The Chancery Court Clerk will also havesigns posted outside its offices with directions on where to go to pick up the settlement check. The settlement checks will be available to picked up starting January 9, 2023, for a period of Thirty (30) days, ending on Friday February 10, 2023, at 4:30 p.m. The Shelby County Courthouse is open Monday through Friday 8:00am to 4:30pm (closed on holidays).

In order to claim your settlement check, all approved Class Members must appear in person and present two (2) forms of identification, one being a photo identification. One of the two forms of identification must be one of the following: (1) Driver’s license; (2) Social Security Card; (3) Passport; or (4) other United States Government form of Identification. In addition, all Class Members picking up settlement checks will be required to complete a form listing his or her current address and social security number. If you are attempting to pick up a check for a person that was approved to receive a settlement check but has since died, please note that you need to go to the Probate Court Clerk’s office (on the bottom floor of the same Shelby County Courthouse building) and complete a Small Estates probate form. The Probate Clerk’s office can assist you in this process. Once completed, you will need to bring the completed Probate paperwork with you to claim the settlement check. Currently, the Probate Court charges $107.50 to open a small estate.
After February 10, 2023, all remaining Class Members that did not pick up their settlement check, will need to appear in person at the Shelby County Chancery Court Clerk, Room 308 at 140 Adams Avenue, Memphis, TN 38103 to claim their checks. After April 10, 2023, the period in which to pick up your settlement check ends, and all unclaimed settlement funds will be returned to the registry of the Chancery Court Clerk for escheatment. This means that, if you have not picked up your settlement check by April 10, 2023, these unclaimed funds will only be available to be retrieved through the State of Tennessee’s unclaimed property procedures. The State maintains an online directory located at “claimit.treasury.tn.gov” to guide you through the unclaimed property process.

There will be no option to have the check mailed, all Class Members must pick up their check from the Clerk’s office in person. The Clerk’s Office will be verifying the identification of Class Members when they claim their check. Each Class Member will be required to swear and/or affirm under oath their true identification before receiving their settlement check. The oath and/or affirmation will be subject to the penalty of perjury.
